Dr. Moazam graduated from Oklahoma University in 2007 in Internal Medicine and Pediatrics. Dr. Moazam treats a wide variety of diagnosis including:

Pediatric - Hematology/Oncology

Dr. Moazam did his residency at Akron Children’s Hospital in 2010 and has been practicing ever since. He deals with various blood diseases including:

  • Thrombocytopenia

  • Cancer survivors

DOT Physicals

  • Dr. Moazam is a licensed medical examiner listed on the Federal Motor Carrier Safety Administration (FMCSA) National Registry. All are welcome to get their DOT physicals at Healthy Horizons Clinic, no appointment needed! 
  • Cost: $50 for patients and $65 for non-patients


Immigrations Physicals

  • Get your immigration physical with us, no appointment needed! Please bring your photo identification, vaccine history, and all other appropriate paperwork. 
  • Cost: $200 dollars for the first visit only. Labs, X-Rays, and vaccines (if needed) will be charged separate

Suboxone Therapy

Suboxone is a prescription medicine used to treat adults who are dependent on opioid drugs (either prescription or illegal) as part of a complete treatment program.
